Yes. Your actual coverage is based on your employer who decides what type of plan your policy will have which is not specific to the type of insurance.
Am I required to pay a co-payment “every time” I come into the office?
It depends on your insurance & what services you are having. Most insurances do NOT require a co-payment, if you’re just coming in for blood work. Many plans require a co-payment when coming in for an ultrasound or an office visit.
Do I need a referral to be seen for a new patient visit?
Most HMO policies, does require a referral from their Primary Care Physician (PCP). We ask that you verify with your insurance company & request a referral from your PCP prior to your initial visit if applicable.
